Dear First Name, Im writing to introduce myself. I am a care services coordinator with The ALS Association Louisiana-Mississippi Chapter, and I working with people living with ALS in your area. As you know, ALS is a progressive and fatal neuromuscular disease that attacks nerve cells in the brain and spinal cord that control muscle movement. ALS leaves patients paralyzed unable to walk, speak, swallow or breathe. Patients typically have a life expectancy of two to five years from the time of diagnosis. There is no effective treatment and no cure. I hope that we can work together to provide better quality of life for those living with this awful disease. As a care services coordinator with The ALS Association, I conduct in-home patient visits, facilitate support groups, manage a durable medical equipment loan closet, and serve as a case manager to assist families living with ALS in taking advantage of the resources available to them. I am also available to educate healthcare professionals on the disease through inservice training sessions. In addition, The ALS Association hosts educational symposiums, offers a respite care program for caregivers, and coordinates a monthly multidisciplinary clinic at Our Lady of Lourdes Regional Medical Center in Lafayette. The ALS Association is the only national not-for-profit organization fighting ALS on every front care services, advocacy, awareness and research. Our chapter currently serves 120 people with ALS and their families in Louisiana. All of our programs are offered at no charge.
