day started off at 8 am -Extraction 15 -extraction 17 -implant 8 -sinus Lift 10 AM: - 67 years old - Implant #8 - removed gum and bone - now sewing gum back together with sheep intestine(dissolvable) - shes been asleep for 1/2 an hour - there was no bone in her upper jaw gumline. so they did a bone graft and then put the tooth back in

10:50AM: -goes back to office and talks to assistant

10:51 AM: -35 years old - wisdom teeth - Puts patient under anesthesia - time out

- everyone has to listen and say they agree, like in hospital - impacted wisdom teeth - once sedated, he numbs them even further with a needle because the patient is not that deep into sleep to do it without numbing him further -Anesthesia: -versed:makes them forgetful - phetnol: narcotic -propothol: keeps them asleep (Michael Jackson died) -has to peel back the bone and gum in order to take the tooth out - its funny when people say that I pull their teeth out, theres nothing to pull!- Dr. Marashi - 3-4 assistants in the room at once - cut the tooth in 1/2 and took out 1/4 - needs to hold patients head back in order to keep the air way open - oxygen mask on the patients nose - heart monitor on the whole time - took out more of the tooth - now theres a hole where the tooth once was, itll now bleed and clot up to make bone and gum - can only pull two teeth bc thats how many the patient can afford ( photographer for car dealerships) - now hes taking out the second tooth

- Jackie keeps injecting his IV to keep him sedated - takes out the other 1/2 of the tooth and keeps cutting at it to take the rest out - then takes out the rest - then sews the gum shut

-19 year old comes in with Dr. Bird for wisdom teeth removal - going to do it next friday and go to school on monday - then grandmother (77) comes in for a consultation. - her upper wisdom teeth need to be extracted, there is very close to the sinus cavity -95% of the time it heals and the results in a hole that he fixes it gives

-Dr. Bird a tour -35 year old wisdom tooth patient comes out feeling loopy and is going to be numb for 10 hours today 11:46 AM: - Tongue lesion patient biopsy -has a defibrillator -Puts in the needle for anesthesia -man goes into deep sleep -puts up the picture of the thing on the TV screen -patient Tries to cough and he moves his hands and legs so they restrain him down so he doesn't move his hands and mess up the procedure -puts in string into his tongue to lift it and to see the lesion

-cutting off the lesion area (removal of lesions right posterior mandible and right lateral border of tongue) -Dr. Marashi kept saying to the patient to stick his tongue out and he kept coughing and moving his tongue around -may be precancerous -puts The lesions in a bottle to get shipped off to get tested -they all wash their hands etc. Assistants ( that I worked with) : -Michelle :24 years old -Traci :Burnett -Jennifer :double piercing Burnett -Jackie :30 years and anesthesia experience Dr. Marashi: What made you want to go into this field? -helping people -working with hands -unique specialty -Only 170 graduate a year -it's a small specialty -broad scope of practice: head neck sinuses rhinoplasty reconstructure -medical school and dental school -15 years undergrad at Mercer -dental school In Minnesota

-medical school at LSU -training half a year at LSU half a year at Rochester, New York -group practices -now solo -He taught at LSU: his specialty he trained residence and he got paid by the school -it was very hard -general surgery is very hard 80- 200 hours a week for residency -Weekends/calls, oncall -stay in the hospital -harder and longer hours all the time -1984 started undergrad his major was biology then pharmacy then dental then went to dental school and was exposed to oral surgery and then became an oral surgeon -came to US in April of 84 and started college in August -went To American school in Iran -He met his wife, Rita, at the time during residency -he was 29 and was married by 31 -during Medical school he had Cameron(16) and Sara (13) -it was very hard with the family and medical school -after medical school he went one year as a general surgeon then he taught at LSU -then went to Charleston West Virginia for a yr -Then came to Charlotte North Carolina in 2002 for 10 years Average day:

Come to work at 8 AM Surgery in the morning consults and follow up in the afternoon Trauma and infections at any time still on call at the hospital at CMC Pineville he does facial trauma, infection, oral he finishes around 5 PM Monday Tuesday Thursday Friday he's in the office Wednesday he's in the hospital he's going to open the second office in May Internet advertising and commercials are expensive six months cost $20,000 he stopped expensive business Dr. marketing Etc: Fun staff likes what he does excited to come to work every morning seven people working: three full-time assistance two full time in the front one part time billing one anesthesiologist and all that is very expensive he has to take out referrals for lunch because they give him patients.

Overall the environment was very friendly the assistants are very nice Dr. Marashi has a very good relationship with all of his assistance it was fun so sit in and watch
