Casus
RvO: Pertrochantere femurfractuur links

72-jarige vrouw, gevallen in de badkamer, waarbij op de X-

bekken een
pertrochantaire femurfractuur links is aangetoond.

B/ lange gamma-nail

Post-operatief is op de afdeling vergeten Fraxiparine

(tromboseprofylaxe) te geven.

24 uur na de operatie kreeg mevrouw pijn bij ademhaling en

dyspnoe.

- X-thorax: heldere sinussen, geen infiltraten
- Lab: D-dimeer 6,45 mg/L (↑)
- CT-angio: verminderde perfusie RMK en ROK

 Een longembolie veroorzaakt een
verandering in de V/Q ratio
 Indien er sprake is van een kleine
embolie, heeft het lichaam een
compensatiemechanisme beschikbaar
om ervoor te zorgen dat de longen
minder lucht ‘nodig hebben’
 Vergeet nooit patiënten na een
orthopedische ingreep Fraxiparine te
geven post-operatief!
