PREVENTITIVE SERVICE SAVINGS AGREEMENT Technician ________________

Inspect, clean, lubricate and make necessary minor adjustment to the equipment, as per manufacture recommendations, during normal
working hours using WNG employees or persons supervised by us. This agreement includes furnishing oils, grease, lubricants, filter
media and cleaning materials. WNG will provide priority repair services at no additional charge.

• This maintenance service will be performed ______________ times per calendar year for __________ years.

• The cost of preventative maintenance service is $_____________ for __________system inspections plus tax.

• All service calls related to equipment repairs or failures are subject to an $89.95 diagnostic fee for a written diagnosis.
Diagnostic fees may be applied upon a signed authorization to perform work from property owner or present operations
manager, based on our current flat rate pricing system. Value rates will apply to all performed repairs and services at the below
stated location and listed HVAC equipment.
