CQI: PROPER WARD ENDORSEMENT Introduction: Nursing shift endorsement is the moment where a nurse gives a report to the

next shift. It is in oral or written account in a face to face exchange. Within this span of time this provides the continuity of care for the patients providing a summary of patients need and details of care to be given. Purpose of the Study: The purpose of this study is to evaluate the nurses accuracy in endorsing patients data and status. With this study, we are able to determine the complete and correct ward endorsement. Also, to eliminate lapses in giving care to patient.

Goal of the Study: The goal of the study is to achieve 100% accuracy in endorsing patients data and status. Methodology: 1. Head nurses of each station evaluate their staff nurses through a checklist. Analysis: 1. The data gathered for 26 random nurses showed that out of 26 nurses, 50% of the nurses are incompletely endorsing patients data and status (13 nurses). a. Out of 13 nurses who incompletely endorsed patients data and status, 62% incompletely endorsed the patients present status. b. Out of 13 nurses who incompletely endorsed patients data and status, 38% incompletely endorsed the patients present status. 2. The Balloon Tree analysis showed that Solutions Formulations: 1. Heads of each station will gather and discuss about the correct and complete way of endorsing patients data and status. 2. Heads of each station will provide a checklist for all the staff nurses as their reviewer and guide upon endorsement.
