Anterior Hip displacement

What is anterior hip


displacement?
• Anterior hip displacement is when a
trauma causes the head of the femur
to pull out of the acetebulum and
move anterior and up.
How does anterior hip
displacement occur?
• The dislocation occurs when forces
transmitted along the femur force
the head of the femur out of the
acetabulum.
• This occurs when the thigh is in a
position of abduction and external
rotation.
• A motorcycle accident is a
characteristic example where the

• The hip is a ball and
socket joint. Made up
of the femoral head
(ball) and the
acetabulum (socket).

• The fibrocartilaginous
acetabular labrum and
transverse acetabular
ligament stabalize this
joint.
Ligaments:
• The Iliofemoral ligament attaches to anterior
inferior iliac spine and acetabular rim and
intertrochanteric line, it provides anterior
reinforcement of the joint.
• The Pubofemoral ligament provides inferior and
anterior reinforcement of the joint, it attaches to
obturator crest of pubic bone and merges with
fibrous capsule of the hip joint on the femur.
• The Ischiofemoral ligament gives posterior
reinforcement and attaches to ischial part of
acetabular rim and attaches to the greater
trochanter and Screws the femoral head medially.
Important muscles:
Movements of the Hip joint
• Lateral or external rotation.
• Medial or internal rotation.
• Extension.
• Flexion.
• Abduction.
• Adduction.
Pathological anatomy:

• In anterior hip dislocation the femoral


head is pushed anterior to the
coronal plane of the acetabulum.

• This causes extreme pain and the hip


and knee will be flexed 90 degrees,
and the leg will be held in external
rotation.
Types of anterior hip
dislocation:-
• anterior obturator:
The femoral head displaces medially
and lies in the obturator canal.
• anterior iliac:
The femoral head moves superiorly
and lies over the iliac wing.
• anterior pubic:
The femoral head moves inferiorly over
the pubic ramus.
Treatment:-
• Reduction must be performed to
return the head of the femur into the
acetabulum.
• Patient will be given anti
inflammatory medication to control
the pain.
• Physiotherapy treatment will be used
to strengthen the muscles and
relieve pain.
