Registration requirements 1. A certified copy of Birth Certificate 2. Current Immunization Form # 3231 3. Ear, Eye, & Dental Form # 3300 4. Documentation of Chicken Pox vaccine or proof of having had Chicken Pox 5. Previous records and test scores (if applicable) 6. All applicable fees Registration, Technology & Book Fees, Membership Fund, Student Activity Fee 7. Applicants must have a letter of good standing and attendance report from school previously attended 8. Acceptance into John Milledge Academy is based on availability of classes, ability to offer classes to meet students needs, past grades, achievement test scores and transcript. 9. Registering your child does not guarantee placement 10. A student is not officially accepted until all necessary documents have been received & approved.
