ensure application for PWD at age 17.5 has been submitted ask family if bank account has been opened encourage family and student to visit CLBC services, ie. day programs, personalized employment programs encourage family to use the last year of Autism funding for transition support, ie. preemployment skills, job coaching, community recreation opportunities

The purpose of transition planning for youth with special needs is to identify opportunities and experiences during their school years that will help them better prepare for life as an adult (Johnson, et al. 2000).
