This is to notify organizations that effective May 4, 2007 amendment 003 is issued to
make the following changes:

1. Change Request for Proposal due date on Amendment #002:

FROM: May 4, 2007

TO: May 18, 2007

2. Change Proposal Guidelines on page 14 of the RFP Item D to read:

The contract period will run from July 1, 2007 through June 30, 2008, contingent upon
Funding availability. The contract renewal to cover program year 2008-2009 will be
dependant upon performance.

Delete statement “Further, GWIB reserves the right to award a contract on

factors other than price.” on page 4, paragraph #2.

3. Change Paragraph labeled Provider Staff Qualification on page 7 of the RFP:

From: The qualifications of staff need to be included in the proposal. The

qualifications must include, but are not limited to each staff member’s
qualification for his/her position, educational background, and licensure if
applicable. In addition, the proposal must make clear any family relationship
that exist within the provider’s staff and disclose same in the proposal and
thereafter is such relationship should thereafter exist.

as follows:
