Assertive Outreach Team (AOT) The Assertive Outreach Team provides specialized treatment plans for each individual

client, including strong collaboration between the client and their community resources, natural support systems, as well as behavioral and primary health care providers. AOT is based at a community level with teams consisting of; medical staff, social workers, therapists, case managers, family advocates, and peer specialists. The clients needs are evaluated, and a custom treatment plan is developed that is focused on providing immediate basic needs such as; benefits, medications, counseling, and housing. As the patients stabilize, the plan adjusts to help our clients to continue to cultivate life skills requisite for independent living.

Centralized Adult Intake Centralized Adult Intake provides initial screening and assessment for new clients, who are then referred to the most appropriate treatment, whether inside Valleys Health programs, or a community provider. Centralized Adult Intake is focused on matching each client with the treatment that best suits them. In addition to reviewing clinical needs our intake coordinators also verify payment options and obtain the necessary authorizations. In order to optimize client follow through our intake coordinators work to set expectations and identify barriers to further treatment, such as; transportation, cultural or linguistic issues, and the stage of readiness for exchange. This decreases confusion by providing one point of access.

Outpatient Clinics Valleys outpatient treatment programs emphasize short term treatment to help individuals and families manage their illness while continuing to live in their community. Treatments include diagnostic evaluation, individual, family, and group therapy, medication evaluation and management, and crisis intervention. Outpatient clinics are located throughout the Salt Lake Valley to provide services to clients near their homes. Community Treatment Program (CTP) I dont see any need for a change, its pretty concise. Clubhouse Programs The clubhouse model employs a strengths based approach, placing emphasis on each persons abilities and talents. The clubhouse programs are designed to build self-esteem, develop work skills, complete educational goals, and help members return to productive employment in the community and to obtain affordable housing. Alliance House Alliance House is certified by the International Center for Clubhouse Development and is one of more than four hundred clubhouse programs worldwide. Clubhouse development is based on the principle that meaningful work plays a crucial role in bringing positive changes to the lives of those living with mental illness. Members pursue educational goals through the Salt Lake City School District and will also work in various positions at the Alliance House as well as other employers in the community. Valley Villa Finding affordable housing is one of the primary efforts of this organization. The Alliance House manages the Valley Villa housing units in Salt Lake City, in order to provide low cost housing for its members.

Substance Abuse Treatment Valley Mental Healths Alcohol and Drug (A&D) Program is designed to meet the individual needs of men and women of all ages. Our goal is to help clients successfully recover from addiction and enjoy a life free from substance abuse. Substance abuse often accompanies mental illness, and all of our programs provide access to A&D resources. We offer a variety of services including outpatient, intense outpatient, day treatment, and residential programs. Needs-specific programs include individual and group therapy, genderspecific programs, services for the elderly, programs for the homeless, and services for those with criminal records. A&D FIRST Program Families in Recovery Staying Together (FIRST) provides a variety of outpatient and daytreatment options for adults, dependent children, and families challenged by substance abuse and mental illnesses. Providing an intensive focus on parenting and rebuilding relationships, FIRST offers assistance as parenting classes, medication management, medication-assisted treatment, and evidence-based trauma treatment. Cottonwood Family Treatment Center

A residential treatment facility is available for women with dependent children and expectant mothers, who are experience life-disrupting substance abuse problems. Typical duration of say is 4-6 months and we focus on supporting the women as well as their children and families.

Alternatives to Incarceration (ATI) Program Sixty percent of all individuals currently incarcerated have a mental health diagnosis. Valley Mental Health has developed a number of programs that support those in the criminal justice system who are challenged with mental illness, substance abuse and/or co-occurring disorders. The ATI program was developed to help individuals obtain services designed to improve their readiness to reintegrate into community life and decrease future involvement in the criminal justice system. We focus on person-centered planning and provide access to a comprehensive continuum of services. Alternatives to Incarceration (ATI) Transport With a goal of reducing the recidivism rate of mentally-ill offenders, ATI Transport connects those sentenced to the Salt Lake County jail to a treatment provider immediately upon their release. Individuals identified and diagnosed as Severely & Persistently Mentally Ill (SPMI) will be ordered by the court to be released to Valley at the time they have completed their commitment. Valley transports these individuals these individuals to a facility with an onsite pharmacy to fill prescribed medication and establish connection to a treatment provider prior to being released back into the community. CORE (Co-Occurring Reentry & Empowerment) Valley Mental Healths Co-occurring Re-entry and Empowerment (CORE) team offers services to adult male criminal offenders with co-occurring disorders, who are offered a 16-bed facility providing wrap-around services both onsite and in the community. CORE integrates mental health and substance abuse treatment approaches, with the ultimate goal of preparing participants for successful reentry into the community and reduced recidivism.

Forensics Program We offer programs for clients on parole/probation who have been referred into treatment by the criminal justice system to help with recovery. Programs offered by the Forensic Unit include mental health outpatient services, domestic violence, substance abuse, and intensive outpatient services; both of which are offered in both English and Spanish. Some of the other services we provide are -Correctional Addiction Treatment Services (CATS)/Oxbow Jail -CATS Outpatient Program -Federal -Mental Health Court -Weekend Warrior Program (WWP)

Fresh Start Program Fresh Start, a drop-in day program run by Certified Peer Specialists, creates an opportunity for those living with psychiatric disability to improve their quality of life. This is done by magnifying individual strengths and skills, while reducing barriers to recover and encouraging the prospect of living independently. This program, which is unique in Utah, provides adults challenged by mental illness a place where they can socialize, learn skills, and share their talents with other clients. Our Certified Peer Specialists (CPS), supported by a staff of trained mental health professionals, share their personal stories of challenge and triumph to connect with clients on a very personal level.
10. Flex Care

The FlexCare Program is a service specifically designed to help clients move out of nursing homes to more independent living. The program employs nursing/social worker teams to provide an array of services designed to keep individuals as safe and independent as possible. Since 2000, FlexCare has helped more than 1, 500 individuals successfully move back into the community. FlexCare is the largest provider of case management and budget assistance services for those on the New Choices Waiver.

With locations in Salt Lake City and Bountiful, the FlexCare program is available to residents of Salt Lake, Davis, Weber, Box Elder, Summit, Tooele, Utah and Wasatch Counties. FlexCare works with a clients medical doctors to coordinate needed services. The program also coordinates with Mental Health and substance abuse counselors and assists with transportation to meet both medical and non-medical needs.

Pheasant Hollow Providing alternative housing for long-term Medicaid clients with physical disabilities who would otherwise be placed in nursing homes. STARS (Supervised Treatment & Residential Services) An assisted living residential support program that provides medication management and mental health services to nursing home eligible individuals. Helps patients who cant care for themselves because of medical and psychiatric disabilities. STARS is a 24-hour group living environment providing room and board for 16 clients.

11. Homeless Programs Valley Mental Healths Homeless Programs serve individuals ages 18 and older who are homeless and have a severe and persistent mental illness (SPMI) which generally includes a psychotic disorder. We work closely with other community resources such as the Fourth Street Clinic and the Salt Lake County Housing Authority in order to support this population. Valley Storefront

Valley Storefront is a non-traditional outpatient unit designated to provide services to SPMI homeless individuals that may also have a co-occurring disorder such as substance abuse. Valley Storefront acts as an entry point into treatment for those individuals who have a resistance to engage in traditional mental health treatment.

Safe Haven Transitional Housing Safe Haven I provides 25 transitional housing units for SPMI patients that have been homeless at least three of the last six months. Safe Haven is based on the Housing First model, which focuses on moving homeless individuals (or households) immediately from the streets or homeless shelters into their own apartments. Residents of Safe Haven are able to maintain their status of homelessness, thereby continuing to qualify for permanent housing offered only to homeless individuals. Safe Haven Permanent Housing Safe Haven II provides 24 permanent housing units for homeless individuals challenged by a history of mental health and substance abuse issues. They are assisted with apartment living, medication management, benefit management, skills development, socialization, and a peer support group that meets monthly and discusses issues that are important to them.

Valley Homefront Permanent Housing Valley Homefront is permanent housing with eight apartments for homeless individuals who generally do well with little supervision, and plan to stay for years. Residents receive in-home visits at least twice a week, attend a tenants meeting monthly, and identify issues and gather as a community.

Shelter + Care - Voucher Program The Voucher Program is a collaboration between Valley Mental Health, Salt Lake County and West Valley City housing authorities. It offers our Medicaid clients subsidized housing and supportive services in the community through Shelter + Care vouchers. We serve nearly 150 clients on various HUD subsidies.

12. Housing Programs

Valley Mental Health owns and operates a number of different housing units throughout the Salt Lake Valley in order to provide affordable housing options for those with mental illness. Housing programs range from independent living situations to supervised housing. These programs primarily focus on skills training and case management to help individuals challenged by serious and persistent mental illness realize their recovery goals.

The supervised housing complexes are listed below:

Oquirrh Ridge East - 12 residents This small apartment complex provides case management, services, and skills development with minimal supervision and a moderately low frequency of contact Oquirrh Ridge West - 12 residents Transitional housing with the expectation that those who develop independent living skills and stable treatment will move to more independent housing.

Valley Crossroads - 20 Residents Transitional housing with the expectation that those who develop independent living skills and stable treatment will move to more independent housing.

Valley Plaza - 72 residents Cooperative living apartments providing 24-hour supervised housing for severely mentally ill individuals. Services offered utilize a specialized, recovery based, interdisciplinary team of professionals who provide a variety of services.

Valley Woods - 56 residents Permanent housing for those with mental illness. Patients are likely to live at Valley Woods for more than two years. Residents are encouraged to increase their independence and move into the community.

Independent Living Program Serving those with a moderate need for structure or contact, but have not developed the necessary skills to live in the community independently. Utilizing the case management model, this program helps the patients to develop skills in food planning, housekeeping, personal hygiene and socialization with neighbors, landlords, and other community members.

13. Specialized Rehabilitation Services (SRS)

Specialized rehabilitation services provide medication management and group and individual therapy services for clients who reside in nursing home facilities. The goal of the SRS program is to improve and promote the overall health of the mentally ill

by working side-by-side with primary care providers and extended care facility personnel.
