The application form obtained from the Circle office, to be duly filled in. Three passport size photographs; one photograph duly attested by Gazetted Officer/Member of Parliament/area MLA/Municipal Councillor. Rs.25/- as non-refundable fee. Proof of Residence A) i) ii) iii) iv) v) vi) vii) viii) B) i) ii) iii) iv) In case of house owner Registration deed of the house in the name of house owner Allotment letter of house in the name of house owner Power of Attorney for purchase of house Attested photocopy of House Tax Receipt for the house Attested photocopy of Electricity Bill for the house Attested photocopy of Water Bill for the house Attested photocopy of landline telephone bill (MTNL) Attested photocopy of bank statement giving the address of the house given in the application form. In case of a tenant No Objection Certificate from the house owner Rent Receipt Attested copy of bank statement giving the address of the house given in the application form If the house owner does not give his NOC and any of the above documents are not available, the area Inspector will obtain a certificate from any two of the neighbors stating that the concerned person stays in the house mentioned in the application form. Old Ration Card Surrender/Deletion certificate in respect of his/her old ration card In case the ration card has been stolen, an FIR registered with the nearest Police Station. Electoral Photo Identity Card School Leaving Certificate or College Certificate If none of the documents are available, an affidavit to the effect that the person does not have any ration card till date, attested by a Notary/Oath Commissioner AND the Area Inspector will obtain a certificate from any two of the neighbors stating that the concerned person stays in the house mentioned in the application form.
