Dear OSHC Parents,

During the course of a year, we have a number of secondary and university students seeking
permission to complete a range of assignments at our school.

In nearly all cases, the school provides permission for this work to be conducted provided that
parental permission is gained and the secondary and university students abide by the school’s
privacy guidelines.

Matt Chambers, a casual worker at the OSHC program, is seeking permission as part of his Monash
University course, to develop a Patterson Lakes Out of School Hours Care blog which will be posted
on the internet. It will be part of the school’s website.

The purpose of this blog is to advertise what goes on at OSHC and provide the students at OSHC a
creative outlet that they can all be a part of and share with their friends.

By allowing your child’s photograph to be taken you agree that it can be used to
1. Be posted on the Patterson Lakes Out of School Hours website
2. Be used in the background/ layout of the website
3. Be used to advertise the website (such as a flyer, poster or email)
4. Be submitted as part of an assignment for Monash University
5. Can be commented on by other children of the OSHC program (all comments will be heavily
moderated and any inappropriate comments will be deleted)

Please note no personal information and/or identification of any pupil other than their first name may
be contained in educational/advertising/promotional material or a school website, whether in
conjunction with a published photograph or not. Please also note that websites can be viewed
throughout the world and not just in Australia where Australian law applies.

I hereby give the Patterson Lakes OSHC program permission to publish, distribute and/or display
work samples and photographic images of my child/ren taken in relation to the 5 above listed
activities.
