Nursing History: Last 1994, the patient was diagnosed of having Diabetes Mellitus .

She had been using Insulin injection for five years already. Last 2013, she experienced blurring of vision and had her check-up and was diagnosed of Diabetic Retinopathy as a complication of her DM. She had her laser surgery of the eyes at the same year. She also experienced epigastric pain for few weeks and was also diagnosed of Cholelithiasis last year. She was supposed to have her surgery last year but refused because she said she was afraid of what might happen during the surgery. Last January 2014, she was admitted for 3 nights at Dr. Uy Hospital due to epigastric pain because of her gallstones. Few hours after discharged at Dr. Uy Hospital, the patient experienced epigastric pain with episode of vomiting of mucus thus prompting the admission.
