Assembly and Analysis


2004-2008: Mercer Health Assembled and analyzed Emergency Room, Doctor Cares, Inpatient charts, and Outpatient Orders. Put physician deficiencies in computer. Processed charts: Reviewed charts for completion. Took completed deficiencies for physicians out of computer.

Filing and Retrieval


2004-Present: Mercer Health Put incomplete charts in physician folders for completion File pictures in permanent file by medical record number File OB charts by delivery date Retrieve charts when requested by staff within the hospital 2004-2005: Mercer Health Did medical records purging in an offsite storage area for all charts where the patient was at least 23 years old and the chart was at least 10 years old as stated in the facilities storage policy.

Prepping and Scanning


2004-2005: Mercer Health Prepped all charts for scanning Made separator/barcode sheets to put before each different part of the chart Scanned all charts that I prepped. Did a spot check on the charts that were scanned to check for errors

Coding
2004-Present: Mercer Health Coded Emergency Room, Doctor Cares, Outpatient Orders 2005-Present: Mercer Health Coded all Inpatient charts and Wound Care charts

Electronic Medical Records Activity


2004-Present: Mercer Health Use M2 for all Medical Records that are electronically done. Use Wellness for all scanned documents

Patient Registration
2004-2005: Mercer Health Worked in the patient registration area and registered patients o Checked patients insurance status Worked as the facilities operator o Transferred phone calls to desired department

Statistics
2005-Present: Mercer Health Do the inpatient statistics for the facility

Clinical Data Abstracting in the Tumor Cancer Registry


2005-Present: Mercer Health Go through all pathology reports for the facility to figure out which ones need to be abstracted in the cancer registry Abstracts all cancer cases in the registry for the facility Submit all cancer cases quarterly to the state Do follow-up on the patients

Development of queries for inpatient coding


2010-Present: Mercer Health Worked one-on-one with DocuComp consulting company to see where the company was losing money due to lack of documentation Developed queries for inpatient charts Put queries on inpatient charts for physicians to document additional diagnosis or further clarify the diagnosis
