Dear Parents and Guardians,

Our class will be taking a field trip to the Michigan Science

Center on Wednesday May 21. As a part of our life science unit, we
are excited to visit the Live Butterfly exhibit. We will be leaving at
10:00 and returning at 2:00. We will be eating a sack lunch,
provided by the school, at the science center. In order for your
child to attend this field trip, this form must be completed, signed,
and returned to class by May 15
th
.
I am also looking for parent volunteers to help chaperone this
field trip. If you would like to be a chaperone on the field trip,
please indicate your interest on this release form as well.
