Student: Billy Bob Joe Ray, Esq.

Source of Referral: Parents
Reason for Referral: Parents called school counselor concerned about Billy Bobs future.
Unfortunately, Billy Bob is underachieving and is failing most of his core classes. He fails to
complete the majority of his assignments and does not spend much time on school commitments
outside of football. Billy Bob is an All-American center for the football team and local colleges
have expressed interest in providing him a scholarship. The colleges will not offer him a
scholarship without a minimum 2.5 G.P.A. His current G.P.A. is 1.85. He will not be able to play
football for the high school next year without a 2.0. Furthermore, Billy Bob does not have any
career interests other than football. His parents are selling the family farm after he graduates so
Billy Bob would not have farming as a post-graduation option. They want him to develop more
future options and realize the danger of relying on a career in football that presents a small
likelihood of attainment.
Goal: Billy Bob will work toward raising G.P.A. to 2.5 and increase academic commitment. He
will learn more about his interests, skills, and opportunities available besides football.

Short-term objectives:
1.) Billy Bob will commit at least one hour to school work or studying every day after school.
2.) Billy Bob will acquire peer tutor.
3.) Billy Bob will research potential career choices based on interests and abilities.
4.) Billy Bob will turn in 95 percent of all class assignments and homework.
5.) Billy Bob will develop system for managing school and extracurricular activities.
6.) Billy Bob will understand the probability of attaining career choices.
7.) Billy Bob will commit to taking SAT or ACT in 11
th
grade
Counseling Interventions:
1.) MBTI and Strong Interest Inventory will be conducted and analyzed to help Billy Bob
determine how his personality and skills can apply to future career choices.
2.) Individual counseling will be implemented to assist Billy Bob in developing a plan for his
short-term and long-term goals.
3.) Coordination and communication of system support such as peer tutors and teachers will
be applied to assist in academic achievement.
4.) Research will be conducted to determine if additional resources such as an IEP are
necessary. Counselor will provide resources for SAT/ACT prep.
5.) Progress of goals for studying, meeting with peer tutor, planning, and completing
assignments will be tracked by providing monthly worksheet to counselor.
Evaluation: Plan will be terminated upon meeting the following objectives
1.) Billy Bob takes SAT/ACT.
2.) Billy Bob provides one-page summary of three career options relevant to his skills and
personality.
3.) Billy Bob maintains 95 percent completion of class assignment and homework for one
semester.
4.) Billy Bob increases G.P.A to at least 2.2.
5.) Billy Bob can provide statistics on probability of becoming a professional athlete.
6.) Billy Bob demonstrates success in balancing school and extracurricular activities by
providing a planning tool or resource.
7.) Parents provide signed monthly log showing Billy Bob spent no less than 20 hours
committed to school work
