This is a narrative report of the incident involving Aldrin Coquia, a student of La Salette
School having been allegedly injured around the school premises yesterday, June 18, 2014. At
around 1 PM I arrived at the school premises and headed straight to the Coordinators Office.
After a few minutes, Mr. Orly Dy approached me and asked for my assistance in rendering care
to a student (Aldrin Coquia) as it was reported that he had sustained a cut and Mr. Dy was
asking my suggestion in the necessary procedures for the child. We then agreed amongst
ourselves (Mr. DY and I) that the student required inoculation of Anti-Tetanus Vaccine as a
precautionary measure. Before heading off to obtain the said vaccine, we queried the student
as to where the injury supposedly occurred. He brought us to the playground area and pointed
somewhere around the bamboo hut though the student was unbale to clearly specify where
the injury occurred as it ensued that the student was basically unaware that he had been
injured until was far off from the playground area. From then on Mrs. Lor arrived and offered to
drive us to the City Health Office for the Antit-Tetanus Vaccinea. From there, the student was
seen by the on-duty physician who advised us that the childs injury should be sutured and
instructed us to transfer to another hospital since they were lacking in equipment for suturing
in the center. After the consultation we then decided to go to Luzon Medical Center for
suturing; once we arrived, the consultant present concluded that the students injuries did not
warrant suturing but a simple wound reduction with cloth plasters were adequate. Not long
after, Mr. Coquia, the father of the student joined us and after a few minutes, the two doses of
prophylactics, ATS and Tetanus Toxoid was given, the student was sent off with home
medications and Myself, Mrs. Lor and Mr. Coquia returned to the school and Mr. Coqiua was
briefed on the incident. The student then went home with the father shortly.
