A Financial or Technical Assistance Agreement application shall be filed in the Mines and
Geosciences Bureau (MGB) Regional Office (RO) concerned for mineral and non!mineral

fee and submission of eight ()) sets of the FTAA proposal and fi"e (*) sets of the follo+ing: a$
