AtSt.CatherineofSienaCatholicSchoolwebelievethattechnologycan
beapowerfultooltoenhancelearningenablingstudentstoaccess
informationandcollaboratewithothersacrosstheschool,community,
andtheinternet.Webelieveallstudents,staff,andparentsmustbegood
digitalcitizensbyfollowingstandardsofacceptableusewhenusing
eitherschoolownedtechnology(software,hardware,network),ortheir
ownpersonalelectronicdeviceforschoolpurposes.

Thisdeclarationisadocumentthatoutlinesthestandardsofacceptableuseforstudents
choosingtobringinapersonaldeviceoruseofanyschooloffereddevice.Oncesigned,itwill
actasanagreementbetweenSt.Catherine'sSchool,thestudent,andhis/herparent(s).

Thisagreementmustbereviewedandsignedbyboththestudentandtheirparent/guardianin
orderforstudentstouseschoolissuedcomputers,iPads,andlaptops,aswellastheirown
personalelectronicdevicesincludingcellphones.Thisagreementisvalidandineffectforthe
currentschoolyearasreflectedinthedatesignedbelow.Thesedevicesmayonlybeusedinan
approvedclassroomsetting.

BysigningthisagreementI,thestudent,agreetoabidebyallschoolrulesregardinguseof
technologyandacceptablepracticesregardingthesame.Iwillnotusemytechnologyfor
anythingotherthanapprovedactivitywithintheschool.IacknowledgethatIamresponsiblefor
myactionsinregardstotheuseofthistechnologyandassumefullresponsibilityandliabilityin
regardstodamagethatmayoccurtomypersonalpropertywhileoncampus.Iunderstandthat
St.Catherine'sisinnowayresponsiblefortheft,damageorlossandIassumefullresponsibility
formytechnologywhileoncampus.

Iagreetoneverusetheabovedevicestobully,harmorotherwisecausediscomfortto
myfellowstudents.
Iunderstandthatmyuseofthisdeviceissubjecttothediscretionofmyclassroom
teacherandwillabidebyanyandallrulestheymakeinregardstothesame.
Iwillutilizethistechnologyonlyfortheexpresspurposeofbetteringmyeducational
experience.
IunderstandIamonlypermittedtousethisdevicetotakenotes,organizemydayto
dayobligationswhileatschool,andcompletehomework.Iwillnotattempttoaccess
theinternet,socialnetworkingwebsitesorapplications,peertopeerfilesharing
softwareoranyothersoftware,appsormedianotpreapprovedbytheclassroom
teacher.Iwillnottake,play,createordisplayvideos,music,soundclipsorpictures
whileinclass,unlesspreapprovedforaproject.Thisincludesmusic,video,and
photographsthatmayalreadybeonmydevice.
AtnotimewillIusethistechnologyanywherebuttheclassroomsthathavebeen
approvedforitsuse.Whenitisnotinuse,itwillbestoredinmydeskorcabinet.This
technologywillnotbecarriedaroundschoolwiththeexceptionoftoandfromapproved
classes.
WhileusingthistechnologyIwillabidebytheSt.Catherine'sCodeofConduct,always
acknowledgingthatitismyresponsibilitytoactappropriatelyandmakewisedecisions.

IunderstandifIbreakthisagreementthefollowingconsequencesmayresult:any1stoffense:
detention/any2ndoffense:lossofpersonaltechnologyprivilegesfortheremainderofthe
quarter/any3rdoffense:lossofpersonaltechnologyprivilegesfortheremainderoftheschool
year.Ultimatediscretiononpunishmentwillbeuptotheadministration.

PersonalTechnologyAgreement.Icertifythatmychildhasreadtheaboveagreement.Iagree
thatbysigningthis,bothIandmychildareresponsibleforabidingbytherulesandobligations
putforthinthisdocument.Iunderstandthatitismyresponsibilitytoworkwiththeschoolin
monitoringtheuseandcontentofmychildsdevice(s),includingcellphoneandsocial
networkingapps,solongasitisbeingusedfortheapprovedactivitiessetforthinthisdocument.
