The assessment should be discussed with the pre-service teacher and carried out by the Mentor
teacher at the completion of the placement. lt must reflect the standard the pre-service teacher
exhibits at the end of the placement. lt is helpful to list any special qualities or abilities the pre-
service teacher has.
ln order to complete this form, please tick the appropriate columns using the following key:
lN ADDlTlOlVplease write comments on the final page outlining the pre-service teacher's
strengths and areas needing further attention.
The assessment form should be signed and dated by the mentor teacher and the pre-service
teacher.
The assessment for the placement will be finalised on the basis of the mentor teacher's form, the
pre-service teacher's own reflections and submitted written requirements and comments provided
by the University advisor.
