Patient Assessment
Cardiac Assessment:
Chest pain (angina)
History of heart disorders (MI, CHF, Valvular)
Other body systems (Respiratory, PV)
Personal habits (wt., diet, salt, meds, tobacco, drugs, exercise)
Psychosocial factors:
Marital status, occupation, education, economics, personality type, general state of
health
Physical Assessment
Apical pulse (PMI @ mitral area)
Retraction, Lift, Heave, Thrill
Tachycardia = >100 BPM
Bradycardia = < 60 BPM
Pulse Deficit = weak, ineffective contraction of L ventricle.
Impulse is not perfused
Ectopic Beats = abnormal impulses that originate outside
normal conduction pathyways
Heart Block = Block in normal conduction pathway
Physical Assessment
Auscultate heart rhythm:
Dysrhythmias / Arrhythmia abnormal HR or irregularity
of rhythm
Bigeminey or Trigeminy irregular rhythms that occur in a
pattern (q 2 or 3 beats)
Irregular irregularity sporadically or with frequency &
disorganization (atrial fibrillation)
Sinus arrhythmia gradual increase or decrease in heart
rate correlated with inspiration & expiration
Heart Sounds
S1 = first heart sound
S2 = second heart sound
Abnormal extra sounds:
Ejection sounds (clicks) r/t deformed valvular opening
heard in systole
Opening snap heard with opening of stenotic valve in
diastole
Pericardial friction rub leathery/grating sound heard with
pericarditis
Murmurs: turbulent blood flow, grade I-VI, valvular &
septal defects
Gallops: S3 & S4 heart sounds r/t a noncompliant ventricle
during diastolic filling
Heart Sounds
Variation for the Older Adult
Thickening and hardening of valves
Decreased cardiac reserve
Fibrosis of conduction system
Difficult to find apical impulse
Increased HR
Premature beats
Murmurs
Angina arteriosclerosis of coronary arteries
