from the CTC will have to be reviewed by faculty member
determined by the Associate Dean and Credentials office.

Student may have additional requirement above the Level II or
Option 4 or 5 program requirements needed to actually receive the
Level II or Option 4 or 5 credential.

Please forward all these to the Credentials Office by e-mail with
student name and ID# and what the student would like to do prior
to the student completing any of the programs.

Credentials Office will have the appropriate faculty and Associate
Dean review students file and then inform the student and Advisor
of any additional requirements.

Special Ed with a
General Ed Credential
For students with a General Ed credential, the Credentials
Office does not need the form anymore that is for your
reference only.

Credentials Office will automatically review and process for all
newly admitted students.

The form is not a guarantee that all coursework will be waived
and students and advisors should check evaluations prior to
designing the ed plan.

All courses waived will need to be done as transfers for a
Masters degree program or the units that were waived
needed to be made up by the student by other course work.
