This term I have demonstrated

professional responsibility in a variety of
ways. I have worked with my partner to
redesign the iThink campaign and make
our own lesson plans for presenting to the
class. I have consulted with my preceptor
in order to establish the best possible
methods for presenting to the
demographic, as well as learning from
what he has experienced with other
projects. I have used a variety of
evaluation techniques that allow for not
only empirical measurements to be
gathered but also qualitative results of
both the effectiveness of the program as
well as our educating techniques.
I have held myself accountable for the
tasks requiring completion, as well as for
my role within my group, the health unit
as well as a member of the Trent nursing
community.

I have created lesson plans that focus on

teaching methods that have been effective
for our age demographic. We have used
interactive activities in order to help the
students apply their knowledge in a

practice when working with community
organizations and the population with whom they

practical manner that will make the lesson

more effective. In addition to this we have
spoken with the teacher in order to learn
more about the population and how we
could gear our lessons towards their
interests. In addition to this we also
consulted the Ontario curriculum in order
to provide knowledge that would further
the education they are already achieving,
and to also allow the lessons to contribute
to the completion of their learning.
In addition to this all information
presented in the lessons has been
referenced and is from reliable resources.
This demonstrates evidence informed
practice, and allows me to provide more
accurate information to the population.

All relationships that have developed in

the health unit have been forged on
respect and my appreciation of their
diverse knowledge. All language that has
been used within the presentations and
while communicating with both my
preceptor as well as the teacher has been
respectful and professional. I have
respected the boundaries set by the teacher
within her own classroom, and I acted
within those boundaries.
I have tried to provide the most relevant
and age appropriate information to the
youth in order to adapt their needs and
level of knowledge.

Demonstrated knowledge of relational practice in
project development, respectful of group process,
leadership/followership, and collaborative
practice to meet the needs and build capacity of
the population in the given context.

Demonstrated self-regulation through the

development of a project plan and milestones,
learning plan, and ability to independently
seek means to meet goals, reflect, and respond
professionally to feedback to meet learning and
project goals that align with the SONs program
goals and the Community Health Nursing
Standards of Practice in Canada.

I have demonstrated collaborative

participation with my partner, other
students at the health unit, my preceptor,
other health unit employees, as well as the
teacher. I have accommodated the needs
of each individual in order to provide the
most effective and well-organized
presentation possible.
I have used the concept of empowerment
while organizing this project in order to
build the capacity of the youth to think
critically not only of the tobacco industry
but of the media as a whole.
I have worked with my partner in order to
equally divide the work and have been
available to help when needed. We have
worked collaboratively in order to develop
the most effective and interesting
presentations possible. I have sought out
other resources in order to enhance my
own knowledge as well as improve the
youths knowledge. I have sought out and
responded to feedback offered by others in
order to improve my own work.
While looking at the learning plan that I
submitted to the health unit I feel
confident in my achievement of these
learning goals throughout the placement. I
have worked to towards achieving these
goals as a way of increasing my success at
the health unit and in this placement as a
whole.

I believe that I have acquired a great deal of knowledge through this placement, not only relating to the project planning and
implementation but also of the health unit. I have learned the wide variety of opportunities that community nursing has to offer, as
well possible future positions that I may be interested within public health. This placement has opened my eyes to a variety of
opportunities in the future and Im grateful for the opportunity to experience such a welcoming and exciting placement.

1. I have increased my confidence speaking in front of large groups that I lacked before this placement
2. I have improved my own presentation skills, both in the performance of a presentation as well as in the creation of age
appropriate lessons that are effective
3. I have developed better research skills than I had prior to this placement, as I was required to seek out a variety of information
that I did not have to before.

Identify 3 areas requiring further development.

1. I need to become more comfortable seeking out additional opportunities and become more comfortable putting myself out
there to experience these opportunities.
2. I need to improve my long term organization, and set realistic goals for myself that are achievable within a specific timeframe.
3. Though I have developed stronger researching skills I feel that this is an area that I could continue to improve in.
