Goal: I would like to learn 2 different forms of formative assessment, and how to
effectively
Integrate them into my lesson plans, as well as how to alter a lesson or a unit based of
the
results of the formative assessment.

Indicator(s) of Success
I will know that I have succeeded with my goal when I create a lesson plan with these new
forms of assessment into my lesson, and when I have used the forms of assessment to
design the next lesson.

lesson plans of my TA, asking them
questions, and observing each
lesson. I will watch for forms of
formative assessment, as well as
changes in their lesson plans as we
move through a unit.
