16:31 am, - Fire units are leaving the scene. All fire related issues have been mitigated. - DAP
4:35 am. - Regency Energy Partner compressor station was on fire located at the intersection of Hinkley Road
and Route 3006 in Springville Township. Natural gas was venting into the atmosphere, until regency personal
secured the compressor station using the emergency shut down process. The visible fire has been extinguished.
Regency personnel are taking the lead venting further natural gas from the structure and are currently checking

Remarks: At 5:30 AM a request by Bob Bisignani, AERPM to respond to a report of
fire/explosion at the regency”s Auburn Compressor Station located in Auburn Twp.,
Susquehanna County.
Upon arrival at 7:00 AM, fire was extinguished: fire companies and Susquehanna County EMA.
personnel had left the scene, Only a wooden pallet and an air filter were on fire; those items
‘were extinguished by the fire department before I reached at the site, I spoke with Mr. Pete
Mecea, Plant operator and he reported that about 04:10 AM, a sensor in the compressor
building sensed @ fire and the station went into emergency shutdown (ESD). At that time, the
station went into a shutdown mode and incoming and outgoing lines were isolated as well as the
‘compressor units. Site inspected and there was no release of any liquids as a result of the fire
found. Site inspection suspected an explosion; however, the company’s personnel could not
confirm all details. The building did present visual evidence that an explosion may have
‘occurred as the walls were blown out, Mr. Nick Bryan of Regency reported that the fire was
extinguished about 5:00 AM.
‘The building involved in fire/explosion houses 3 compressor units each having 1380 HP
‘capacity. Mr. Jeremy Daniel of Regency arrived at the site at 8:00 AM, Mr, Daniel reported that
they would have their experts on scene in the morning to begin the investigation as to what
happened. No injuries were reported. The company verified that the station was in total
shutdown and would remain that way at the present time until operations could resume safely
requested that operation of the Auburn Compressor Station, including dehydration and
compression, should not resume until the PA DEP NE Air Quality Program has been
coordinated with and provided a written incident report.
Follow up with Air Quality recommended.

Description & Timeline:
4:07am: Regency Gas Control (GC) received multiple alarms, including gas and fire detection,
for the Auburn Compressor Station (station) and that the emergency shutdown valves were
actuated.
4:08am: Regency Operations were notified by Regency GC and dispatched to the station.
4:59am: Regency Operations arrived at the station and confirmed there was a fire; local EMA
and fire department were already at the station. At that time, the gas had been shut off and the
fire was out in the building. Only a wooden pallet and an air fiter were stil on fe; those items
were extinguished by the fire department.
5:03am: Regency GC informed Susquehanna County 911 Center of the status.
18am: Initial notification was made via telephone to the PA DEP Northeast Regional Office
Emergency line by Nick Bryan of Regency.
36am: Nick Bryan spoke with Bob Bisignani of the PA DEP Emergency Response Team and
informed him of the event. Mr. Bisignani indicated that he would be sending an inspector to the
station
5:43am: Nick Bryan spoke with Shailesh Patel, the PA DEP Emergency Response Inspector, to
provide site location data, site contact and summary update.

‘Approx. 7:00am: Mr. Patel arrived at the station. Mr. Patel and Jeremy Daniel of Regenoy
inspected the site. There was no release of any liquids as a result of the fe. The PA DEP.
requested that operation of the Auburn Compressor Station, inoluding dehydration and
compression, should not resume until the PA DEP NE Air Quality Program has been
coordinated with and provided a written incident report.
Corrective Action:
Regency has initiated a full investigation and root cause analysis of the fire. It is expected that
the investigations will continue over the next week and Regency will provide the PA DEP with a
subsequent report, once additional information is available. The Auburn Compressor Station
will remain out of operation until itis determined safe to return to operation and is in compliance
with local, state, and federal regulations.

EXECUTIVE SUMMARY
This summary contains the preliminary findings from an Apollo Root Cause Analysis™ (ARCA™)
conducted in Tunkhannock, PA in regard to an incident that resulted in a fire/explosion that occurred
on the morning of December 1, 2014 at the Auburn Compressor Station facility. Effective Problem
Solving, LLC was contracted by Regency Energy Partners to facilitate cross-functional team of
Regency Energy Partners employees and a third party compressor expert from CDM Resource
‘Management LUC through the analysis.
‘The purpose of the analysis was to determine the causes that contributed to the incident and identify
solutions to control these causes in an effort to prevent or lower the probability of a future occurrence.
The RCA was facilitated by Mr. Dennis Rygaard (EPS) and was conducted utilizing the Apolio Root Cause
Analysis™ method. The results are based entirely upon information provided by representatives of the
above mentioned parties present at the RCA group facilitation and evidence provided by employees
not present at the facilitation. More causes may emerge if new evidence or information is presented.
The preliminary findings indicate that the fire and explosion occurred when an unplanned release of
hydrocarbon vapors (97.5 % methane}: were released and ignited. The unplanned release occurred
during normal operating conditions when'a %” carbon steel schedule 180 nipple on a pressure gauge
assembly broke off of the Unit:One Compressor discharge east scrubber bottle in an enclosed
structure. The gauge assembly was a field installation (non OEM) so operations could monitor
differential pressure in the process. The nipple had been in service since start-up of the facility in
October 2013. The configuration of the assembly (material of construction, size, weight, etc...) and
vibration inherent in compressor operations and/or excessive vibration more than likely contributed to
the failure.
Corrosion from chemical attack’ gn the nipple may have played a role as well however; evidence to
support this will be not be determined until a metallurgical analysis of the pressure gauge assembly has
been conducted. The most probable cause for the ignition source is the exhaust manifold on the unit
which reaches temperatures of 1250 degrees Fahrenheit. The exhaust manifold has some level of
ulation however; its purpose is to provide thermal protection only.
It should be noted that the Emergency Shutdown Systems (ESD’s) in place functioned as designed
Which eliminated a potential for a catastrophic event. There were no injuries as a result of the
explosion, The building sustained major damage however; the equipment upon initial assessment did
not... mechanical integrity investigation of the equipment is in progress to confirm this conclusion.

As noted in our previous report sent to the Department on December 2, 2014, please find the attached supplementel
summary anaiysis of the event that occurred at the Regency Auburn Compressor Station on December 1", 2014. since
then, the station has been shut down in its entirety while cleanup, health & safety, damage assessment, root cause
analysis and integrity teams evaluated the station
Since all facility piping and equipment outside the building were not damaged, Regency wishes to resume free flow of
82s through dehydration only, without compression, as the compressor units have been completely isolated from the
‘est of the facility. Unless the Department otherwise objects, we plan to resume dehydration operation tomorrow,
December S*. During the free flow period, repairs to the building and to the compressor unit on which the malfunction
occurred, will continue over the next several weeks. Once the building & compressor unt repais are complete, we will
re-commission the unit and notify you when we intend to return online with compression,
2s poked inthe inspection report, once we return to normal operation, portable analyzer testing will be promptly
scheduled and completed.
