Please reference our 7-day follow-up letter submitted pursuant to the requirements
of LAC:33:1.3925, dated January 9, 2009, in which we stated that the incident was
under investigation and that additional information would be provided. The
incident investigation was not completed until March 4, 2009 and, due to an
oversight, the additional information was not submitted within the timeframe stated
in the January 9, 2009 letter.

Tank operations at the Meraux Terminal were temporarily takén over by the Meraux

In May of 2009 a procedure was developed by Murphy Oil USA, Inc. Supply and
Transportation for the safe filling of tanks at the Meraux Terminal. The procedure
was issued on June 3, 2009, personnel were trained and in early July 2009 the
Meraux Terminal resumed control of its tank filling operations
In addition, construction began in July on the installation of an ENRAF Advanced
Tank Gauging System on all product tanks at the Meraux Terminal. Installation was
completed in September at a cost of approximately $600,000.00 and commissioning is
expected in October.
‘The ENRAF system is installed such that the signals are monitored and recorded in
the Central Control Room of the Meraux Refinery and in the Meraux Terminal Office
As a backup, Differential Pressure Level Transmitters have been installed on the
tanks. If either device reads a high alarm, the system will alarm both at the
Meraux Terminal and at the Meraux Refinery Central Control Room
WAS THE RELEASE PREVENTABLE?
Yes. The incident investigation completed on March 4, 2009 confirmed that the
release was preventable
