PHARMACEUTICAL CARE IN SURGERY

WORKSHOP ACTIVITY: Inpatient Prescription Chart Review


You are currently providing a ward pharmacy service to Anson, a surgical ward. You are required to
review the prescriptions for the following patients and identify any pharmaceutical issues that need
to be addressed. You are also expected to document actions to resolve these issues
You may refer to any available guidelines on the ward that may be relevant in the care of these
patients:

SURGICAL PATIENT NO: 1


Additional information
Francine Browne, an 85 year old retired university lecturer is admitted from Restwell Nursing home
with a fractured right hip which she sustained after falling out of bed. She is scheduled for a hip
operation to have a dynamic hip screw placed in her right hip. Prior to admission Mrs Browne was
on lansoprazole 15mg ONCE daily and Gaviscon Advance as required for GORD. She has no allergies.
Her eGFR on admission was found to be 25mL/min/1.732.

Additional information
Lucy Bloggs, a 19 year old university student is scheduled for an appendicectomy. The admitting
doctor has prescribed surgical antibiotic prophylaxis on the treatment chart.
