AFFIDAVIT
I, MARIO BIDANG, of legal age, Filipino, and a resident of Brgy. Danac
East, Boliney, Abra, after being duly sworn to in accordance with law, hereby
depose and state:
1. That, I am at present the Secretary of Nagsingisingan Farmers
Association, Inc., a duly registered peoples organization operating
in Barangay Danac East, Boliney, Abra;
2. That, the Nagsingisingan Farmers Association, Inc. is engage in
reforestation activities under contract from the Department of
Environment and Natural Resources National Greening Program;
3. That, none of the incorporators, organizers, directors, officers or
agents of these organization is related by consanguinity or affinity;
4. That, I am executing this affidavit to attest to the truthfulness of the
foregoing facts and circumstances and for all legal intents and
purposes it may serve.
