Patient is a 59 year old Caucasian Female
Lives in Laurinburg, NC
Middle Class
Christian
Occupation/ Education level- Real Estate broker for over 20 years
Completed community college courses at Richmond Tech for Real estate
Marital status/ Married for 41 years
Family status/ 1 son age 36

Problem Statement
Referral from primary Physician to the Cancer Center
The patient was referred to the center due to the fact that she has breast

cancer and 3 brain tumors

Rationale for referral- patient needs to start treatment


Patient accepted her referral

Strengths and Limitations


Strengths: Patient accepts their diagnosis of cancer and is willing to

accept treatment and a hospice referral when the time comes.

Patient remains positive about the overall outcome of her treatment and

she feels that God will guide her through this difficult time.

Limitations: Patient is extremely worried about her friends and family

when the day comes that she will leave this earth.

Assessment goals and Interventions


Assessment:
Client needs chemotherapy and she has no health insurance
Patient cant afford to pay for her prescription of ALOXI
Patient cant afford to pay for gas to get to and from her appointments

Interventions:
Have patient meet with Medicaid representative at the hospital by the end of

today

Register patient today for Financial assistance for her prescription through

the Esai Assistance Program

Check status of application daily


Set patient up for gas vouchers (weekly)

I assumed she would answer questions a certain way due to her diagnosis.

What were some value differences between you and the client?

The client believes that God will make a way for her to be able to pay for treatment and
prescription without assistance and I believe that she needs to receive assistance through the
center and foundation.

What events or life experiences have shaped them into the person they are? Her diagnosis of
breast cancer/ brain tumors.

What is the staff recommendation about diagnosis, Interventions or treatment?


Chemotherapy treatments every Tuesday, take prescription 1x per day.

What does research say on their diagnosis/problem/treatment?

Hard for patients to accept their diagnosis and that they worry about leaving their loved ones
behind.

What changes do recommend for the agency or service (Needs of agency, changes, and rights
issues)? Try to have a onsite Therapist or Counselor
