LEARNING GOAL 1:

Learning Needs:
I need to improve assessment skills for vital signs and pain assessments. I
also need to learn how to use a Glasglow Coma Scale on a resident.
Learning Goal:
I want to demonstrate a full set of vital signs, a PQRSTU assessment and a
complete Glasglow Coma Scale on a resident/patient. I will be able to do
this by April 3 2015.

The practice document this goal relates to would be the Professional


Standards document. It would relate to the standard of knowledge
application, due to the fact that my goal mainly involves the practice and
improvement of a professional knowledge.

my practice?
My learning will train my competency in assessing different clients. This
will also improve my skills of critical thinking as well as communication
with the health care team.

What impact has my Learning Plan had on my practice?
So far I have improved on my skills of assessing vital signs and pain on
clients. I have learned that questions are the main key into assessing and
discovering vital information that may contribute to a clients well-being.
My Learning Plan has given me more confidence in this area and it has
also helped me realized what weaknesses I need to work on.

LEARNING GOAL 2:
Learning Goal:
The Professional Standard that is relevant to my pre-graduate clinical experience
that I have chosen would be the standard of leadership. Based on that, I would
like to learn and improve my sense of communication and collaboration with
clients, as well as the health care team. I would like to achieve this goal before I
end my clinical placement on April 10th.

Activities and Timeframes:

Receive feedback from clinical teacher before submitting documentation


(by April 10th 2015)
Introduce myself to new clients and communicate with them on a regular
basis to develop trust (by April 10th 2015)
Attend and participate in staff meetings (by April 10 th 2015)

My learning will support my commitment to continuing competency by
helping me improve in an area that is very important in the health care
field which is communication. By enhancing my sense of leadership, I will
be able to develop better relationships both interproffesionally, as well as
with clients.

Evaluation of changes/outcomes to my practice: What did I learn?


What impact has my Learning Plan had on my practice?
Throughout this semester, my sense of communication and leadership has
been well enhanced. I would have my clinical teacher review all of my

documentations before they were submitted and as time went by, less and
less corrections needed to be made. My clinical teacher had informed me
that my documentation skills had well improved. I have also been able to
build rapport with residents through efficient communication. By
frequently collaborating with the residents, I was gradually able to build a
healthy nurse-client relationship with them, as well as a sense of trust.
Finally, I had the chance to attend several staff meetings and was even
able to participate in one of them. In that particular meeting, along with
another student, I was asked to state any possible concern that could put
any of the residents involved in my care at risk. I was able to do so without
any difficulty. Overall, my Learning Plan has had a positive impact on my
practice.
