1. What percent weight loss is considered a significant change?


The percentage of weight loss that is considered significant change is 5% change in 30
days or 10% change in 180 days.
2. What is the normal range for fasting blood sugar (FBS)?
The normal range for fasting blood sugar (FBS) is 70-100 mg/dL.
3. How can you check whether a client/resident is becoming dehydrated?
In order to check whether a client/resident is dehydrated or not there are two approach.
One is physical examination that includes checking for vital signs such as reduced blood pressure
or increased pulse rate is a sign of dehydration. Furthermore skin of patient can be checked as
dehydration makes skin less elastic. If a patient is dehydrated then they will have a dry mouth so
tongue can be checked for this. On the other hand, lab tests that can be conducted to check
whether an individual is dehydrated or not include blood tests (electrolyte abnormalities),
Hemoglobin count, BUN, Creatinine or Urinanalysis.
4. How can you know whether a client/resident has fluid retention?
In order to diagnose a client/ resident for fluid retention (oedema) several symptoms can
be checked for instance swelling in face, breast, tummy (abdomen), hands or feet. Moreover
check for weight difference in morning and evening is also a way to check for water retention.
Other symptoms of fluid retention include rapid weight fluctuation or gradual weight gain over
days. One way to physical examine a patient for fluid retention is by pressing skin, and checking
the indent in skin.
5. Name some key sections of the MDS that document nutritional factors.
Some key sections of MDS that document nutritional factors are Section H Bladder and
Bowel, Section K Swallowing/Nutritional Status and Section N Medications.

6. What characteristics would you look for in a client to determine if their health status has
improved?
There are a lot of characteristics we can see to deter if the health of patient has improved
on not such as their vital stats, lifestyle, mental stability, lab reports and their symptoms.
