Complete Survey
Teachers asking 3rd through 6th grade students
the survey questions through a show of hands
and then document the results on the survey
provided to the teachers.

1.In a quiet room ask students to raise their hand if
they hear a
ringing in their ear.
2.Raise your hand if you can think a way to protect
your hearing.
3.Raise your hand if you wear hearing protection,
such as ear
plugs, when you are surrounded by
loud noises.
4.Raise your hand if you think you can regain your
hearing after you experience a hearing loss.
5.Raise your hand if you regularly listen to loud
music.
6.Raise your hand if you are regularly around
constant loud noise.
7.Raise your hand if you have family members with
hearing loss.

MISSION
To provide education
to 3rd through 6th
grade students on
hearing loss
prevention.

GOALS AND OBJECTIVES


Students will learn how to protect and appreciate their
hearing.
Students will be provided with visuals and interactive
materials to instill
hearing protection habits
for life long hearing protection care.
Students will learn ways to overcome peer pressure to not
use or overlook hearing protection
Students will learn to be role models for hearing
protection promotion.
Students will learn these three methods throughout the
program: turn down the volume, use hearing protection,
and/or walk away.

Prevent (Disability Prevention)


Rationale for Using Intervention Approach:
This intervention approach is a good fit
because it focuses on hearing prevention for
students who may be at risk for occupational
performance problems in their day-to-day
contexts and environments. This program
carries out the ideas of this intervention
approach by focusing on the prevention of
hearing impairments.

Evaluate with a survey within
24 hours and with a another
survey after 2 months and
collect data to evaluate
success of retained hearing
protection habits
Adjust program to reflect
needed changes for success in
problem areas.
