Long Term Goal:

The client will improve receptive, expressive, and cognitive skills to improve
communicative abilities in adult daily living (ADL).
Short Term Goals: The client will
1. Point to the correct object when given a verbal description with 80% accuracy
with minimum verbal cueing.
2. Follow 2-step directives when given verbally with 80% accuracy with minimum
verbal cueing.
3. Follow 2-step directives when given verbally, the function of each object with
70% accuracy with moderate verbal cueing.
4. Point to the correct object when given the written name with 70% accuracy with
moderate visual cueing.
5. Verbally recite a written phrase with 70% accuracy with maximum cueing.
6. Choose the correct written response when given the choice of two with 80%
accuracy with minimum cueing.
7. Correctly recite a phrase using Melodic Intonation Therapy to describe a picture
with 70% accuracy with maximum cueing.

Procedures:
1. The clinician will use a pencil, marker, pin, fabric, thread, ruler, and tape in a
verbal description for the client to point to the correct object.
2. The clinician will use the previous materials to have the client follow 2-step
directives when verbalized.
3. The clinician will use the previous objects/ materials and have the client follow
2-step directives when given the function of each object verbally.
4. The clinician will write one of the previous objects/ materials on a dry erase
board and have the client point to the correct object.
5. The clinician will use daily living phrases that the client will use to express her
wants and needs, by having her verbally recite them.
6. The clinician will use those daily living phrases again to have the client choose
the correct phrase, when given a choice of two.
7. The clinician has previously introduce Melodic Intonation Therapy and will use
it in this session, to describe a picture.
8. The client has a communication book to assist her in adult daily living and the
clinician will briefly use it in conversation with client.

Subjective Data:
The client seemed alert and ready for the session. She also was cooperative and
was very pleasant. There were times she showed frustration.

Assessment:
The client is pleasant and willing to try and learn new strategies. The client is still
struggling with most of the goals. They are pointing to the correct object when given a
verbal description; following 2-step directives when given verbally and given the
function of each object; verbally recite a written phrase; and correctly reciting a phrase
using Melodic Intonation Therapy. The client understands pointing to the correct object
when it is written and correctly choosing between two written phrases. The cueing for all
of these varied. The clients communication book is a good tool and easy assistance for
her in ADL.
Plan:
The clinician will continue on current goals. The clinician will assist the client in
working more with 2-step directives. For following a 2-step directive when given
verbally, will increase the cueing to moderate instead of minimal cueing. Since the client
has some facial paralysis, clinician will work on oral motor exercises. Also, introduce and
trial the P.R.O.M.P.T. strategy, which is a tactile cueing system. The acronym stands for
Prompt, Restructuring, Oral, Muscular, Phonetic, and Targets.
