I, ERLINDA N. PUA, of legal age, Filipino, with a business address at No.

166-A Alfonso XIII St., Brgy. Corazon de Jesus San Juan City, Metro Manila,
after having been duly sworn to in accordance with law hereby depose and state
that:

I am one of the incorporators and the duly elected CORPORATE

SECRETARY of GONGPACHI CONCEPTS INCORPORATED, a
domestic corporation duly organized
and existing under and by virtue
of the law of the Republic of the Philippines;

2.

I, in behalf of said corporation, hereby undertake to amend its corporate

name from GONGPACHI CONCEPTS INCORPORATED to
GONGPACHI CONCEPTS INCORPORATED doing business under the
name and style of OGAWA JAPANESE RESTAURANT.

3.

The abovementioned name sought to be added in the corporate name

shall serve as the business name and style to which the aforesaid
corporation seek operate and do business upon the completion of
amending the corporate name;

4.

This Affidavit is executed to attest to the truth of the foregoing and for
whatever legal purpose and intent it may serve.
