4:00-4:05 • Prayer to be led by

4:05 –4:15 • Assigning of patient task to the Staff Nurses
• Checking of Paraphernalia’s
4:15 – 4:30 • Nurse Patient Interaction( building of rapport and
interviews)
• Taking of initial vital signs
4:30 – 5:00 • Chart Review, carrying out of Doctors Order and
updating it on the Kardex and Endorsement Sheet
• Performing Nursing Procedures and Interventions
• Vital Signs taking for 6pm
5:00 – 5:30 • Checking of Sample Charting of the first batch by the
Head Nurse
5:30 –6:00 • BREAK of the first batch; be sure you will endorse your
patient properly before having your break.
• Checking of Sample Charting of the second batch by
the Head Nurse
6:00 – 6:30 • BREAK of the second batch; be sure that you will
endorse your patient properly before having your
break.
• Checking of Sample Charting of the first batch by the
Clinical Instructor
6:30 – 7:15 • Continuation of checking of Sample Charting of the
second batch by the Clinical Instructor
