PROCEDURE : NASOGASTRIC TUBE REMOVAL

PURPOSE : Remove tube as soon as patient can tolerate orally to


minimize complication.

REQUIREMENT

1) Tissue paper.
2) Disposable gloves.
3) Mouth care equipments.
4) Protective sheet.

STEP

1. Verify Dr’s order for removal – passed flatus, gastric or small


bowel drainage is not excessive in volume.
2. Greet patient and explain procedure.
3. Turn off suction if any and clamp tube.
4. Position patient in recumbent / Fowler’s.
5. Place protective sheet across patient’s chest and wear disposable
gloves.
6. Remove tape from patient’s nose.
7. Instruct patient to take deep breath and hold it to close epiglottis.
8. Withdraw tubing slowly and cover it with gauze as it emerges.
Pull tube quickly as it reaches nasopharynx.
9. Provide patient with equipments for mouth care.
10.Make patient comfortable.
11.Clean and clear equipments.
12.Record and report findings.

NURSING RESPONSIBILITIES

1. Ensure procedure is carried out on client at correct site.


2. Reassure client to gain cooperation.
3. Perform procedure in a correct manner to prevent complications.
4. Ensure client’s comfort is maintained throughout procedure.
5. Record findings and report abnormalities.
