IMPORTANT NOTE:
For Students
1.

Students have to fill reappear subjects of 1st, 3rd and 5th semester only .

2.

The students should pay the examination fee of Rs 900/-

3.

The Reappear form should be filled and signed by the student.

4.

The Photocopy of the receipt be attached with the application form.

5.

Students should submit the form in the office of HOD, not later than 10th
Sept. 2015.

For HODs
1. The HODs should submit the completed forms (soft as well as hard copy)
along with a consolidated list (Stating the No. of students per subject)
to Dr. Sanjay Sharma, Room No. 203, Block No. 1 by 21st Sept., 2015.
2. HODs will ensure that subject code & subject title are correct.
