For one selected patient, the student will do an entry in their reflective
journal, to include the following items about their patient:
1. Pseudo-name and patient diagnosis
2. Age, and mental status
3. Very brief medical history to include any co-morbidities
4. Current medications list
5. Living situation (alone, with relatives, assisted living, etc.)
6. One or two pertinent teaching points that could be done with
patient / family / caregivers to enhance or improve selfmanagement
