Please make sure to come to the NAPOLCOM Regional Office/Place of processing on your
appointment date with the following requirements (original & photocopy):
1. Examination Application Form (NAPOLCOM Form 1-A) which may be downloaded from
the NAPOLCOM website (www.napolcom.gov.ph) duly accomplished by the applicant.
2. Two (2) recent and identical 1 in. x 1 in. color ID pictures (with white background) and
complete name tag (indicate rank before the first, middle and last name, if PNP Member).
3. Police Examination Fee: Four Hundred Pesos (Php 400.00)
4. One (1) legal-size window envelope with fifteen pesos (Php 15.00) worht of stamp/s.
5. Transcript of Scholastic Records (with Special Order issued by CHED) or Diploma from
CHED-Accredited Schools, Colleges or State Universities
6. Birth Certificate issued by the Local Civil Registry or by the National Statistics Office
(NSO)
7. For PO1 with Temporary Appointment, a certified copy of attested appointment, plantilla
appointment or absorption order
