Summer Horse and Pony Camp 2015

Please review the following available camp sessions, being sure to note which ones are
age appropriate for your child, and check which session(s) you would like your child to
attend. Each session is Monday-Thursday from 8am to 3pm: Pony Camp age 6-9 Horse
Camp 10-16.

Please calculate your total due. Each session is $300. We require a $100 deposit at the time of registration to hold
your desired spot, the balance being due at least two weeks before the session. HELMETS ARE REQUIRED. If one is
not owned, rental is offered for $8.00 for the entire week.

I understand that the $100 per week reservation fees are registration charges and are
NOT REFUNDABLE under ANY circumstances. I understand that cancellations for ANY
reasons, within 2 weeks of the day camp session will receive no fee remission. Payment
in full is due 2 weeks before start of camp. This is NONREFUNDABLE within two weeks of
the camp date. Session may be forfeited if final payment is not received on time.
IMPORTANT (READ BEFORE SIGNING): I certify that in case of accident or illness, the
camp director has my authority to secure medical attention if unable to communicate
with me immediately. I understand there is some inherent risk in activities at camp and
that accidents sometimes occur. I understand that the camp fee does not include
accident insurance. I agree that after a place is reserved for a child, he or she will
remain until the end of the period unless necessary withdraw due to illness as defined by
a physician. I understand that no refunds are given if a child leaves early due to
homesickness or for disruptive behavior as decided by the camp director. I give my
permission to Four Seasons Equestrian Center Summer Horse and Pony Camp to use
photos or videos of my child in promotional literature.
