The undersigned Police Non-Commissioned Officer, born on January 17, 1955 would
like to apply for optional retirement effective October 2006 pursuant with the provision of
Section 40 of Republic Act 6975.
Subject PNCO was enlisted to Philippine Constabulary on 16 March 1978, absorbed into
the PNP and will be completing his thirty (30) years dedicated service on the above mentioned
effective date of retirement.
The applicant is not scheduled for separation or retirement from the service due to
attrition system for cause, has no pending administrative or criminal case.
This application is made possible for the undersigned to avail optional retirement benefits
granted by the organization, to closely attend to the immediate need of his family.
