The SPE must be implemented and completed as approved by QCI- for the Education program. It is the responsibility of
the student to submit an Application for Assessment Extension form for approval and negotiate mutually
acceptable days to make up any absence with their SPE Mentor and the SPE Site Coordinator.

This SPE Student Attendance Form must be signed by alt relevant persons. It is the student's responsibility to
ensure the correct SPE form is signed and returned with the SPE Final Report at the end of the Supervised Professional
Experience. Failure to submit this form on time may lead to a Grade Pending as this form is required for finalisation of
achievement of a course assessment.
