WHAT SHOULD I DO WHEN?


A. In the event of accidents/injuries: Attach copies of any relevant documents.

What are the schools injury policies? Send the child to the schools nurse
located in the main office or to Ms. Monique
What documents must you complete? An incident report must be completed in
case of an injury
Where is the first aid kit? It is located in the closet in the cafeteria
Who should you contact in the event of an injury? Ms. Monique

B. In the event of fire/evacuations: Attach copies of any relevant documents.

What are the schools fire procedures? Calmly escort all students to grassy
area located near the play structure and wait for an all clear signal.
Where are the fire exits? Office, side gates, and grassy area near play structure
Where is the meeting point? In front of school

C. In the event of unauthorized access of children: Attach copies of any relevant


documents.

What are the schools procedures should an unauthorized individual attempt to


access the children? Call on campus security or Ms. Monique on walkietalkie. Escort all students to cafeteria or room 37 which is the hide out room.

D. In the event of school lockdown: Attach copies of any relevant documents.

What are the schools procedures should a school lockdown be enacted?


Students are to remain in room 37, cafeteria, or restrooms.

ABOUT THE ACTIVITY SPACE:


A. Risk assessment of activity space: Identify all potential hazards in the
OUTDOOR activity space (draw a map of the area and indicated all potential
hazards).
B. Risk assessment of activity space: Identify all potential hazards in the
INDOOR activity space (draw a map of the area and indicated all potential
hazards).
C. Where are the closest restrooms and water fountains?

ABOUT MY STUDENTS:
A. Risk assessment of students medical needs:
- Identify all students with specialized medical needs.
- Be aware of student profile/medication and identify school medical
personnel responsible for these students

PRE-PROGRAM PREPARATION:
A. Where is the equipment located and how will I access it? The equipment
is located in the shed. The key can be provided by Ms. Monique.
B. Where will I go if it rains? Cafeteria
C. How will I make sure the students are adequately protected from the
weather (sun/rain/wind etc)? All activities will be designed based on the
weather updates before arriving to the school.
D. What will I do if a student is not appropriately dressed for Physical
Education? Students who are not properly dressed will sit out for the
remainder of the activity due to possible injuries.
E. Where can I leave my personal belongings? Personal belongings may sit
on the benches outside on the blacktop next to the activity areas.
F. What access, if any, do I have to the teachers lounge/facilities? N/A
Additional Notes:
A fence surrounds the playground. There are 6 basketball courts and two restrooms
located outside. There are 3 water fountains located on the playground and near the
restrooms. When it rains, there are puddles placed sporadically throughout the
blacktop.
