Explain the experience of acute illness in individuals

receiving care in acute setting

-Patient teaching on acute illness

-Week 2: I taught my patient the importance of the sequential
compression devices (SCD) he had on this legs because he would
not wear them. However, once I explained the reasoning and how
these were preventative measures he understood and agreed to
wear them while in bed.
-All weeks: encouraging ambulation in order to promote
healing.
-Answered questions about procedures patients have had
(prostatectomies, below knee amputations) to promote better
understanding of their acute illness experience.

Interpret critical aspects of the persons experience of

acute illness in relation to common signs and symptoms,
responses to treatment, patterns of coping, and impact on
individual and family relationships

-Week 3: identified patient was not coping too well post- In

operatively because I noticed complete lack of motivation progress
S
for self-care. I had to continuously encourage patient to
do things such as shave, bath, go for walks, change gown
and even clean up his suite, which usually patients who
are coping well do without repetitive prompting.

Identify common medical treatments and potential

consequences/complications of selected acute illnesses

-Week 5: Below knee amputation identified possible

complications which may include infection, phantom limb
pain, blood clots, and damage to nerves.
-Week 3&4: Prostatectomy (surgery to removed prostate)
potential consequences blood clots, infections,
rectum/urethra damage, urinary incontinence. Important
to monitor input/output balances, presence of flatus,
bowel sounds, encourage ambulation.

Demonstrate selected nursing and collaborative

interventions related to clinical pathways, peri-operative
care, IV medication administration, cardiac assessment
and rhythm strips, neurological assessment, wound care,
blood component therapy, TPN and central lines,
pulmonary care including chest tubes and tracheotomy,
initiating IVs, rapidly changing conditions, and
resuscitation

-Week 3&4: Administered/hung IV medications (i.e
Zantac) with supervision from clinical instructor.
-Week 5: Wound care and packing of below knee
amputation with clinical instructor.
-Week 3&4: Emptying and monitoring Jackson Pratt
drains.
-Week 3: bladder scans.
-Week 3 & 4: PO medications
-Week 2,3,4: discontinuing IVs.
-All weeks: emptying catheters

b. Work with patients and families to

Critically appraise own practice in relation to nurseclient/family interactions and as a member of the health
care team

-In my own practice my client and family interactions

have been kept professional and respectful. I always listen
to the patients thoughts/concerns/requests and answer
any questions the patient or the family may have. If I am
unsure of the answer I refer them to somebody who can
answer or go inform myself then relay the information.
For example: Once a dressing change had to be done on
one of my patients while a family member was present. I
explained both to the patient and the family the
procedure and what their role as well as mine was going
to be. I answered questions they had about healing, pain
and expectations as well as any other questions about the
specific surgery (prostatectomy).
-I am a good member of the health team I as work
together with not only my peers and my instructor but
also with the other staff on the unit. I assist other nurses
when even when not explicitly asked to and fulfill my
duties as a team member in order to contribute to the
flow and function of the unit.

Participate in professional development based on

reflective practice and critical inquiry

-I have participated in reflective practice through

reflective journaling on experiences I have encountered
that have marked me. For example: An experience of

ethical distress I experienced involving a patient who had

received a kidney transplant but continued to deliberately
expose herself to harmful teratogens such as smoking.
This really made me reflect upon my morals and
understand some of the issues one can come across
throughout their professional career.

Clinical Instructor Comments (All areas marked as unsatisfactory must have a

comment)
Gabrielle, I have given you S in all areas, this is mid-term and there is still lots to learn.
In the next few weeks I would like you to look at blood-work and diagnostics, this will help
you connect the dots.
Keep up the good work!
