Directions: The purpose of this form is to assist the Student Teacher in evaluating
ones growth at the midterm and fnal points of the student teaching experience. Each
candidate is asked to complete the form and share his/her results with both the
University Supervisor and the Cooperating Teacher.

improvement:
I have improved in Domain 1 by writing lessons farther in advance so I then only
have to go back to make minor changes when the time comes. I have improved
my classroom environment by trying not to talk over students and getting their
attention first. I have improved my instruction by finding more variety in the
lessons I teach, although I still need to work on this. I think I have also improved
my instruction in relation to the specific students I teach by figuring out what
works best for them. Keeping up with grades and parents was overwhelming at
first for Domain 4, but definitely have gotten a better handle on this.

Areas within the Four Domains where I hope to demonstrate


improvement:
I want to improve my planning and preparation by planning more
specific questions to ask during lessons. I also hope to improve my
planning of formative assessment, which also falls under Domain 1.
For Domain 2, I hope to become better at getting more talkative
classes to quiet down faster and to establish clearer procedures and
transitions. For Domain 3, I would like to improve the quality of the
questions I ask, my discussion techniques, and student participation.
I hope to make it a habit to reflect on lessons immediately after, or as
soon after I teach it as possible, which is a part of my professional
responsibilities.

MIDTERM: (please complete at mid-term only)


Two short-term professional goals I have set for myself include (goals
you hope to accomplish in the next six weeks):
I hope to add a spark or more creativity to my lessons in the next few
weeks. I have established a good environment with the students, but I need
to be more engaging and creative in my lessons. For example, I want to try
to have my students actually act out the reading of The Crucible. I also want
to explore more project based learning activities.
Another goal I have is to stop asking questions that lead to yes or no
answers. I have made a conscious effort to ask higher thinking questions,
but I still need to work on this. Q&A on the spot can be difficult, so I want to
have lists of questions I can refer to during lessons.
