Present Levels of Academic Performance

Literacy: McCadden can currently read with 80% accuracy on a level B book. He also knows 30
sight words. McCadden will learn 5 new sight words to be able to increase his competency
reading harder leveled books.
Math: McCadden can currently add single digit numbers with sums less than 5 with 80%
accuracy. The next goal is for McCadden to add two single digit numbers with sums up to 10
with 80% accuracy.
Leisure: McCadden currently enjoys playing with a ball at recess. He enjoys playing when
others initiate the exchange of passing the ball. With prompting McCadden will begin to initiate
playing with a ball with same age peers in order to increase social skills and independent
pleasure.

Annual Goals and Objectives

Literacy Goal: McCadden will learn 10 new sight words to use in daily life and reading
situations.
Objective: McCadden will learn 2 new sight words during daily one on one reading group
with 80% accuracy.
Objective McCadden will learn 2 new sight words and practice the previously learned sight
words with 80% accuracy.
Objective McCadden will learn 2 new sight words and practice the previously learned sight
