Council hereby give notice to the Clerk oflhe Council and lhe lvlonitoring Officer for Cheshire
East Council of those disclosable pecuniary interests which I am required to declare under The
Relevant Authorities (Disclosable Pecuniary lnterests) Regulations 2012 and my Town/Parish
(delete as appropiate) Council's Code of Conduct.
I undersland that in so doing I must also declare not only my own interests but also any interest I

Underthe Localism AcI201'l and the above Regulations you must register your pecuniary
interests. lf you fail lo do so, and then participate (speak or vote or both) at a Town or Parish
Council or commiilee meeting, you may be committing a criminal offence.
The information that you give on ihis form is required to be kept in a Registerthat must be
available for inspection by the public. lt musl also be available for inspection on your
Town/Parish Council's website (if applicable) and on Cheshire Easl Council's website so it is
imporlant that you keep it up to date. Gifts or hospitalily received for a value of8100.00 or more
should be included on a separate form.
You musi, within 28 days of becoming aware of any change of circumstances notify the Clerk to
lhe Council and/or Cheshire East Council's Monitoring Officer asking that the information be
included in your Register of Members' lnterests.
The Ac{ provides that if information relating to your interests is "sensitive', that is to say if you
and lhe Monitoring Officer agree that disclosure of ils details could lead you, or someone
connected with you, to be vulnerable to violence or intimidation, it need not be included in the
Register, even if it is a disclosable pecuniary interest. Please consult the Monitoring Officer if
additional information is required on this issue.
