Directions: The purpose of this form is to assist the Student Teacher in
evaluating ones growth at the midterm and fnal points of the student
teaching experience. Each candidate is asked to complete the form and share
his/her results with both the University Supervisor and the Cooperating
Teacher.

I have improved my ability to plan higher order thinking questions and I have
planned some more creative lessons than I was doing before.
Domain 2:
I have improved my ability to establish a clear routine and expectations for the
students when they come to class.
Domain 3:
I have gotten a lot better at being clear about asking questions and finding
multiple ways to ask the questions so students know what I am looking for.
Domain 4:
I am a lot more confident in my ability to make the right decision and in my
relationship with parents of students. I have gotten better at keeping track of
missed assignments and scheduling times for them to be made up.

Domain 1:
Although I have made an effort to step up the creativity of my lessons, I still have
a lot of room for improvement. There is nothing wrong with my lessons, but they
could be more engaging and unique. This requires more research when planning.
Domain 2:
I still need to work on better transitions and really making the most of instructional
time. I want to make my transitions faster and more productive.

Domain 3:
My questioning techniques have improved, but I really hope to get even stronger
in this area. I want to ask critical thinking questions and be able to facilitate
discussion amongst the students with less talking form me.
Domain 4:
I want to work even more with other teachers to learn better techniques and ideas
for lessons. I also want to join a professional organization.

One or two long-term professional goals I have set for myself include
(something you want to improve within your frst year of teaching):
My long-term professional goal is to continue reflecting on lessons and to add
creativity to my lessons. I hope that within my frst year of teaching I am able
to fnd exciting and successful ways to engage students. I want to do this by
continuously reflecting and not getting stuck in a simple routine. I know it will
be easy to become used to doing things one way and I want to make sure that I
avoid this. My frst year of teaching will be a huge learning experience, but I
want to make it a goal to continue this learning even after I am a teacher with
several years experience behind me. This means keeping up with professional
development, constantly consulting with other teachers, and joining
professional organizations.
