As of today, I have not received any certificates indicating your

participation in at least two of the workshops that included in the list that
was attached to your Teacher In Need of Assistance plan. You have also
failed to provide me with notes that you drafted from observing Mrs.
Browns class. Both of these directives were due, in writing, by January 30,
2015. In addition, you have not collaborated with Mrs. Brown to develop
lesson plans that were student centered and engaging. The deadline for
these lesson plans were every Monday, one week prior to the assignment
day. This memorandum is to inform you that you have not performed the
directives that were assigned to you in your Teacher In Need of Assistance
plan.
By signing below, I declare that I have received a copy of this
memorandum. My signature is not an admission of agreement with
this memorandum. I understand that if I have any discrepancy with
this memorandum, I have ten business days to respond in writing.

Subject: Non-Compliance with Teacher in Need of Assistance Plan

On February 3, 2015 I observed your fourth period biology class from 11:05am until
11:55am. During this class period you taught 24 students about the different stages of
the cell cycle. Your approach to teaching this topic consisted of you reading through a
PowerPoint for the first 35 minutes of class. After you got through the PowerPoint you
instructed students to independently write a one page summary of what they learned
about the cell cycle. I noticed a few students raised their hands to ask a question to
which you responded:
Wait until I am finished and I will answer your questions.
Another student had their hand raised for approximately 2 minutes without you being
aware because your back was turned to the class while you were lecturing. There was
evidence of a lack of student engagement. I witnessed 5 students on their cell phone
during the class period. One student was asleep in the back row.
While the students were writing their one page paper you stepped out of the classroom
and were absent for 5 minutes. Throughout the class period the students were not
offered any sort of involvement in their own learning and were left with questions that
you failed to answer at the end of your lecture.
According to the directives set before you in your Teacher In Need of Appreciation your
goal is to Create a positive and engaging learning environment for your students by
using enthusiasm and positive interaction with the students. In the 50 minutes that I
was in your classroom I did not see any interaction with the student other than to direct
them into their assignment. The students were not engaged and environment did not
reflect a positivity.
Your list of goals and self-evaluation was due to me in writing by February 20, 2015 and
that deadline has lapsed.

This memorandum is to inform you that based on your Teacher in Need of Assistance
Plan you have failed to fulfill the goals and directives that have been assigned to you.
By signing below, I declare that I have received a copy of this memorandum. My
signature is not an admission of agreement with this memorandum. I understand
