Oar pamise enous rg December 14, 2015 Dear Sir or Madame: Diya Aravind is a 7 years old, with recurrent fsgs status post renal transplant who could potentially benefit from lipoaphersis. In short Diya was diagnosed with FSGS at 2.5 years of age. After 6 years she went into end stage renal disease and received a renal transplant on May 8th 2015. Unfortunately, the disease recurred (this happens in approximately 1/3 of cases of pts with FSGS who receive a transplant. She has undergone multiple treatments to alleviate her recurrent disease which will unfortunately if untreated led to the lost of her new transplanted kidney. This has included plasmaphersis and several doses of rituximab without any response. To that end she may benefit from lipoaphresis, using the liposorber machine. Unfortunately, this technique and machine is not available in India. In contrast we have been using this technique here at Nemours/A.|. DuPont Hospital for Children for the last 6 months with some positive results in patients with FSGS. Therefore, | request that Diya be allowed to travel to A.l. DuPont hospital to undergo a 9 week course of lipoapheresis. This therapy may be life alternating for Diva and has the potential to help save her renal transplant. Diya is scheduled for her first appointment on January 20, 2016 for line placement to start the lipoaphresis on January 22, 2016.
